Rates and codes

This is what replaces the pile of coupon codes. Prices live in a table, and each discount carries its own eligibility and stacking behaviour, so the rule is enforced by the system rather than by whoever the code was emailed to.

Early-bird pricing is a column in this table rather than a coupon code, so it cannot be handed out early, used late, or accidentally combined with another offer.
